1. Describe your progression as a clinician including key roles and experiences.
   Since becoming a clinician, I have fulfilled various roles in order to gain the necessary
   experience to be the best clinician possible. I have worked in inpatient settings, outpatient
   settings, and have even done some home visits. I have also had the opportunity to work with
   a diverse population of clients, which has allowed me to learn how to effectively
   communicate and work with people from all walks of life. My progression as a clinician has
   been steady and I have gained the skills and knowledge needed to be successful in this field. I
   am constantly striving to improve my clinical skills and remain up-to-date on the latest
   research so that I can provide the best possible care to my clients.

 Senior Clinical Educator: Educate clinical staff on necessary skills/ techniques, policies,
and procedures to complete their daily tasks.

 Vice-Chairperson of Hospitality Committee in HANA (Haitian American Nurse
Association) – The role and scope of the Hospitality Committee shall be to ensure that all
who enter into contact with the Association are made to feel welcome, treated with
respect, courtesy and consideration and is valued as an essential part of the Association.
The members of the Hospitality Committee are called upon to:

1. Reflect on how our meeting can best greet and welcome visitors and newcomers.
   Members will serve as official hosts, welcoming members and guests at all Association
   functions.
2. Prepare and arrange refreshments for Association functions.
3. Provide hospitality arrangements for invited guests (hotel accommodations,
   transportation assistance).

 Health Department Director at Philadelphia SDA Church: Provide weekly health
education sessions to church members such as prevention and management of common
diseases and disorders. I also served at the church clinic to help those in need while they
are on the premises.

3. Describe volunteer activities within the past two years and time spent on these activities.
    Community Health fairs: organized and participated in health fairs within my work
   community. Provided education on high blood pressure, diabetic management, and
   HIV/AIDS assessed vital signs and administered the covid vaccine to family members of
   patients living in skilled nursing facilities.

 Youth Pathfinder Club: Served as Master Guide in rank. It is a church-based para-
military-like movement for youth from 10 to 17 years of age. I provided them with basic
life surviving skills such as sewing, cooking, cleaning, knot tying, CPR/first Aid,
swimming, and much more.

 Church Community Service: Organized and participated in back-to-school activities by
providing school supplies to children in the selected community; provided hair stylists
and barbers to prepare them for their first day of school.

6. Describe how your area of clinical interest aligns with the DNP specialty track and
   how you made the decision to pursue this specialty. *

Since I became a nurse 18 years ago, I have been committed to making a difference in the
life of all that I would care for. Nursing is my passion and the profession that I’ve dreamed of
since childhood. As an RN, I have the privilege to care diligently for my patients during their
period of illness. There is no greater joy than knowing that I participated in rendering care
resulting in patients’ best health outcomes or providing the best palliative care to those with
poor health prognoses. As the years went by, my curiosity to learn more and eagerness to
perfect my skills remain all-time strong. I decided to pursue the DNP at the University of
Florida because its program aligns with my clinical needs as its focuses on preparing
professional nurses with the knowledge and skills necessary to practice using new methods
and ideas based on the best evidence in an industry that is constantly changing.

7. Describe an area of clinical interest you would like to explore further as a clinical project.
   Why does this area interest you?
    Cardiovascular Health Education post-discharge. This area interests me because heart
   disease is the leading cause of death in the US. A Healthy heart
   Is central to overall good Health.
   It is estimated that one in three adults in the United States has some form of
   cardiovascular disease. Cardiovascular disease is the leading cause of death in the United
   States. Despite these statistics, many people do not understand the importance of taking

care of their cardiovascular health. After a person is discharged from the hospital for a
cardiovascular event, it is essential that they receive education on how to take care of
their heart. This education should include information on diet, exercise, and medication.
It is also important for people to be aware of the signs and symptoms of a heart attack or
stroke so that they can seek medical attention immediately. Cardiovascular disease is a
preventable disease. By making lifestyle changes and taking medication as prescribed,
people can greatly reduce their risk of having another cardiovascular event. It is
important for people to understand their risk factors for cardiovascular disease and to take
steps to reduce those risk factors. With proper education and support, people can live
long and healthy lives after a cardiovascular event.

8. Describe your personal and professional experience that has best prepared you for
   doctoral study.
   I am a nurse with over 20 years of experience in a variety of settings. I have worked in acute
   care, primary care, and public health. I have also taught nursing at the community college
   level. My experience as a nurse has given me a deep understanding of the complexities of
   health care. I have seen firsthand the importance of evidence-based practice and the impact
   of policy on patient care. I am passionate about improving the quality of care for all patients,
   and I believe that pursuing a DNP will allow me to make a greater impact in this area. I am
   committed to lifelong learning and to using my knowledge and skills to improve the health of
   my patients and the nursing profession. I am confident that my experience as a nurse has
   prepared me well for doctoral study and that I will be an excellent nurse leader.

   Clinical Educator- I love to share my knowledge with others.
   There are a number of things in my background that will uniquely contribute to the
   community in the College of Nursing. First, I have a strong academic record that
   demonstrates my commitment to learning. Second, I have a diverse set of experiences that
   have allowed me to develop a variety of skills. Finally, I have a strong desire to help others,
   which I believe will be a valuable asset to the community in the College of Nursing.
