Camden Management Health Initiatives

The National Quality Strategy (NQS) seeks to improve the quality of care that whole
populations receive through encouraging adherence to six priorities. These priorities are not only
meant to improve quality of care that patients receive but also reduce the cost of care. Various
healthcare facilities have adopted initiatives that are in line with NQS in order to achieve its
objectives of providing quality healthcare at affordable cost. Here is a look at how the Camden
Coalition of Healthcare Providers meets some of NQS priorities through their management
initiatives.
One of the coalition’s initiatives is to have patients work with a community-based team
that comprises of health coaches, community health workers, social workers, and nurses
(Camden of Healthcare Providers (CCHP), 2017). This initiative is in line with NQS’s second
and third priorities which seeks to involve patients and their families involved in their care as
well as promote coordination and communication of care providers.
Another initiative is imparting patients with skills and support that helps them care for
themselves in a manner that makes them avoid hospital readmissions (CCHP, 2017). This
initiative is in line with NQS’s fifth priority which encourages working with communities to
promote best health practices.
As the video of Dr. Jeffrey Brenner shows, Camden also seeks to reduce healthcare costs
through application of unique targeted care such as conducting home visits to persons who
whose healthcare costs are high (Brenner, 2011). This is in line with the sixth priority which
seeks to have affordable care for patients.

Even though most of initiatives meet the priorities of NQS, there are still areas that the
program can improve on. One of these areas is researching and promoting the most effective
prevention methods. Apart from improving quality of care, prevention methods also reduce costs
of care.
The model employed by NQS and Camden use population health approach. This
approach seeks to improve the health of the entire population through reducing health
inequalities and making sure that health intervention measures address a broad range of factors
that influence the health of patients.
