There are various important components of medical history. Medical history is
comprised of the identification details of the individual. These details include the name, the
address, the date of birth and the contact information. Medical history may also comprise the
next of kin and the date of birth. Government-issued identification number may also be
indicated in the medical history. In addition, medical history contains a summary of the
medical information (American College of Sports Medicine, 2018). Medical information
includes any previous diagnoses, findings of physical examination and any symptoms that the
individual may have exhibited in the past. A medical history details the history of the
symptoms such as palpitations and heartbeat sensations. It also details recent illness,
hospitalization and surgical procedures (American College of Sports Medicine, 2018). Drug
allergies and medications that an individual is using are detailed in the medical history. The
exercise history, work history, and family history are also indicated in order to give the
relevant history that can affect the health of the patient.
Purpose of the Informed Consent Form
Informed consent is an important ethical and legal consideration. While the design of
consent form may vary, the form should capture enough information to capture enough
information to ensure that the participant has a clear understanding of the risks associated
with the test or exercise (American College of Sports Medicine, 2018). Consent helps the
patient to asks questions regarding the exercise program as well as ensure that the individual
understands the risks as well as the benefits of the exercise program. Informed consent also
helps in outlining the responsibilities of the participant. The participant should have the
ability to report any unusual symptoms relating to the exercise process (American College of

Sports Medicine, 2018). The participant also has to report any medications including
nonprescription to ensure that the exercise program runs smoothly.

Components of an informed consent form include:
Purpose and Explanation of the Test: the section explains the test that will be conducted as
well the underlying reasons why the test will be conducted. It also explains that the intensity
of the exercise program will begin at low levels and progress to advanced levels based on the
fitness level of the patient (American College of Sports Medicine, 2018).
Attendant Risks and Discomforts: the section seeks to expound on the risks and
discomforts that could result from the exercise program.
Responsibilities of the Participant: The section outlines the duties of the participant in
respect to the exercise program and health conditions. It gives the participant the
responsibility to report signs and symptoms relating to exercise (American College of Sports
Medicine, 2018).
Benefits to Be expected: the section describes the benefits that would accrue the participant
as a result of the exercise test.
Inquiries: It gives the participant the right to ask question on procedures applied in the
exercise test.
Use of Medical Records: It outlines the protections accorded to the medical records obtained
during exercise testing (American College of Sports Medicine, 2018).
Freedom of Consent: It gives the participant the right to engage in the exercise test
voluntarily.
